I agree with you, using the case management approach will help reduce the rate of readmissions. Advantages of Case Management include: promotes interdisciplinary collaboration, increases quality of care, eases patients transition from hospital to community services (Black, 2017). Do you think presenting the benefits of having case management will help change management position?
